Policies are an important part of ensuring that the expectations and scope of care and services rendered are clear for all parties involved.

  • Dr. Luber is an out-of-network provider.

    She provides assistance in obtaining reimbursement via your insurance company’s out-of-network benefits by providing you with a detailed invoice for all services rendered. The client or responsible person is ultimately accountable for all fees due to Dr. Luber.

    It is recommended you reach out to your insurance company to learn about reimbursement and possible deductibles before scheduling an appointment.

  • Dr. Luber reserves the right to charge the full session fee for sessions that have not been cancelled within the 24-hour window before the scheduled day and time of appointment.

  • On-time payment is expected for services at the time of the visit.

    Credit card, personal check, or venmo are acceptable forms of payment. In addition to weekly appointments, the same hourly session rate applies to ad hoc professional services.

    If your account has not been paid for more than 30 days and agreement for payment has not been agreed upon, legal means may be used to secure the payment.
